A foreign transaction can happen in two ways:

Currency Conversion
If a purchase is settled in a currency other than US dollars, it counts as a foreign transaction—even if you're in the U.S. when you make it. -

Visa defines this as a transaction where the card issuer is in a different country than the merchant. For example, buying something online from a foreign company while in the U.S. is still considered international.
